So looking at the manual you can try to coop with people while setting a password to your game. Is it possible to play the game fully in coop with an actual friend 100% of the time this way?

Yes. But summoning a player for coop automatically summons an invader as well.

I could be wrong, but I don't see an option to adjust neck size. There are a lot of options to adjust the face, but only a few for the body.
