It's entirely likely the XI staff know what the players and former players want. The survey is a tangible set of evidence they can use to negotiate more resources. "X amount of formers players say they will resub if we do X" type thing.
I would not be meek or considerate in your requests. Ask for what you really want and give honest feedback or the exercise is pointless.
