It's a desktop customization tool called rainmeter which you have to run like any program after you have booted up http://rainmeter.net/cms/. You get the program, choose which skin you want (or combination or skins; I have features of 3 different skins going on) then mess about with them until it looks awesome!

The background is not included, just a cool background I found that goes very well with my setup ^_^

I may actually start using that, would get rid of the last icon and keep the functionality :D
