It takes effort to become morbidly obese, maybe being over weight you can blame it on a slow metabolism but hitting the morbid line happens because we don't take proper care of ourselves. BMI also doesn't account for muscle weight so if you are at all built then what it tells you becomes more and more BS the more built you are (so basicly its built with unhealthy to middle of the range people in mind)

to be fair, i'm not sure that pumping blood through 220 pounds of muscle is much better for your heart than pumping it through 220 pounds of fat
